24 Hour Emergency Assistance
Covers on-call counseling and problem-solving and/or immediate response for assistance at a person’s home due to a health/personal emergency. Whenever the person needs other services, the support planning team will meet to revise the plan to meet the person’s assessed needs.

Adult Companion Services
Adult Companion Services provide companionship and support to help individuals participate in social, recreational, and community activities. Services are designed to promote independence, social connections, and meaningful participation in the community based on the individual’s needs and preferences.
Homemaker services
Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.
Night supervision
Provides overnight assistance and monitoring by an awake staff in the person’s home.
Respite Care Services
Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care. Respite provides the level of supervision and care that is necessary to ensure the person’s health and safety.
Individual Community Living Support (ICLS)
ICLS covers assistance and support for eligible people age 65 and older enrolled in the Alternative Care (AC) program or the Elderly Waiver (EW). It includes the following service

Community living engagement
Individualized Home Supports (IHS)
Individualized Home Supports is a one-to-one service that provides opportunities for people to increase their independence and connect with their community. Support can include assistance with daily living activities such as eating breakfast or brushing teeth to participating in local events and building relationships with friends.

Individualized Home Supports With Training
Individualized Home Supports with Training provides individualized support and training to help people develop and maintain skills needed for daily living, independence, community participation, and health and safety. Services are tailored to the person’s needs and may include guidance and hands-on support with household tasks, personal well-being, and other activities that promote greater independence.

Individualized Home Supports with Family Training
IHS with Family Training provides support and training to individuals and/or their family members to build skills needed for daily living and to help the individual remain safely and successfully in their home. Training may focus on areas such as daily living, household tasks, community participation, and health and safety.
This service is available to children and adults living in their family’s home and is focused primarily on teaching skills that increase the individual’s and/or family’s ability to provide and maintain support at home.

PCA/CFSS Services
PCA/CFSS provides personal care and support to help individuals with daily activities and maintain their health, safety, and independence. Services may include assistance with personal care, daily living activities, household tasks, and other eligible supports based on the individual’s assessed needs and service plan.

Employment Exploration Services
Employment Exploration Services help individuals explore their interests, skills, and abilities to identify potential employment opportunities. Services may include learning about different types of jobs, exploring work environments, developing employment-related skills, and gaining experiences that help the individual make informed choices about future employment.
